THE RELATIONSHIP BETWEEN FIBROMYALGIA AND OBESiTY
Carrying excess body weight increases the level of pain and the extent to which widespread pain occurs in people with fibromyalgia. It also increases the severity of other symptoms of this chronic pain syndrome. Studies have shown that obese people with fibromyalgia are not so physically strong or flexible, have more sleep problems, experience greater levels of anxiety and depression and experience lower quality of life than ‘healthy’ people with fibromyalgia.
Akiko Okifjui, PhD, a researcher at the Pain Management Centre at the University of Utah in Salt Lake City says the bond between obesity and pain is strong. “Research shows that obesity increases the severity of symptoms and pain sensitivity in almost all chronic painful conditions, including fibromyalgia and osteoarthritis”, she says. A 2015 editorial about the pain-obesity link, published in Pain Management, has for example reported that people who are overweight (BMI between 25-25.9) have 20% more chronic pain than healthy people, while obese people (BMI) between 30-34) or very obese people (BMI between 35-39) experienced 68% and 136% more pain respectively.
Painful strain
Strained joints are part of the pain-fibromyalgia relationship, says Okifuji. She says, “The more you weigh, the more it puts pressure on your joints, especially in the lower parts of your body. This can negatively affect your pain level”.
This finding was confirmed in a 2010 study, published in the ‘Journal of Pain’. In short, obese people experience an increased sensitivity to pain, compared to people with lower weight, especially in the lower part of the body.
“This extra sensitivity in the lower part also points to mechanical stress as the cause of pain in fibromyalgia”, concludes Akiko Okifjui. Overweight is a form of inflammation “Cells in the adipose tissue are chemically active and release a number of proteins and other chemicals that cause inflammation throughout your body”, says Okifuji.
“Researchers begin to compare obesity with a normalised state of constant low grade inflammation”, she says. “Although fibromyalgia is not an inflammatory disease such as rheumatoid arthritis, increased general inflammation is likely to play a role in increasing pain and sensitivity.”
WEIGHT LOSS IMPROVES EFFECTIVE FIBROMYALGIA SYMPTOMS
A 2012 study, published in ‘Clinical Rheumatology’, compared individuals with fibromyalgia who reduced their BMI by about three points, enough to move the majority of them from obesity to the overweight classification, with other subjects whose weight remained stable.
People in the weight loss group showed significantly improved scores for inflammatory proteins and almost all fibromyalgia symptoms, including pain, depression and sleep disturbances.
